![]() Title: Job Coach Hours: part time Start Date: As soon as possible Description: A Job coach is responsible for assisting persons with a disability in maintaining work in the community. It is the job coaches primary responsibility to assist persons with a disability in learning the skills necessary to maintain community employment. A Job coach will assist individuals in learning new job tasks, assist in helping people acting appropriately on the job site, and assist in making sure transportation and other issues are taken care of so the individual may succeed in community employment. Qualifications: Ability to work well with people, knowledge of disabling conditions helpful; good oral and written communication skills; customer service experience required. Must possess a valid Driver’s License with clean driving record. Must pass a criminal background check. Must be able to work flexible hours, including weekends. Duties Include: 1. Learn the appropriate job coach techniques in assisting individuals with disabilities in maintaining employment. 2. Complete job coach training by Rehab Services, Inc. 3. Become knowledgeable in disabilities of the individuals served and how that may impact their work performance. 4. Become knowledgeable of medications utilized by individuals to determine how this may interfere with their ability to complete job tasks. 5. Develop an individualized plan of service with the client and with the Employment Specialist. 6. Assist clients with completing job tasks utilizing the job coach training. 7. Do job coaching/co-worker model training on the job site with clients. 8. Keep employment specialist informed on client’s job performance. 9. Follow-up with employers and consumers on placements. 10. Attend team meetings held for specific clients. 11. Complete monthly summaries and narratives for each client. 12. Attend employment staff meetings when required. 13. Complete MIS forms and Remittance sheets for program billing. 14. Complete termination (close-out) materials for clients when services end. 15. Complete necessary reporting forms. 16. Assist in orientation of new and present staff. 17. Complete other duties as assigned. |
